Housing is the foundation for health and behavioral health. It is a critical component in the lives of the individuals we serve at the Department of Behavioral Healthcare, Developmental Disabilities and Hospitals (BHDDH) to ensure stability, create community and support goals such as employment, social connectedness and recovery.
The Department is excited to offer this Housing Retention Tool Kit in collaboration with our partners at Horizon Health Partnership (HHP) and the BHHP Training Group. This tool kit was created with input from case managers and property managers to assist frontline staff in working with their clients to help them keep the precious resource of the new housing they have obtained. We hope you find this tool kit helpful and appreciate any feedback you have on how this toolkit can be more helpful to you.
